Targeted Case Management Services provide assistance to individuals and their families in developing, planning and implementing supports that provide meaningful direction in their life. The Support Coordinator works closely with the individual and their family or representative to assist them in designing a system of support and services that enables the individual to increase and maintain their independence through Community Inclusion, Choice, and Empowerment. The Cass County Board of Services funds Casco to manage the Targeted Case Management Services.
